Lateral C2--C3 subarachnoid puncture for metrizamide myelography or cisternography.

چکیده
Metrizamide is useful for evaluating both posterior fossa and the cervical spinal canal [1, 2]. Because the contrast material is diluted by spinal fluid enroute from the lumbar region to the cervical spinal canal, the C1-C2 puncture technique has been advocated as the most suitable approach for instillation of contrast material [3]. The procedure is safe and without great technical difficulties, although problems with a puncture at this level have been reported with lesions centered at C1-C2, [4]. There is also thepossibility of injury of a herniated cerebellar tonsil or anomalous caudal loop of the posterior inferior cerebellar artery. In patients with basilar invagination or congenital fusion deformities of the occiput and upper cervical spine, a C1C2 puncture may be impossible. Thus, a lumbar puncture remains the safest approach in patients with possible ton-

Iohexol versus metrizamide for cervical myelography: double-blind trial.
Cervical myelography was performed by lateral C1-C2 puncture in 60 patients. Thirty patients received iohexol (an investigational aqueous contrast agent) and 30 received metrizamide in order to provide a prospective, randomized, double-blind trial comparing these contrast media. The two media produced radiographs of equal quality. Contrast-related morbidity was suffered by four patients (13%) i...
متن کاملComplete myelography with metrizamide.
Thorough myelography of the entire spinal cord including the foramen magnum region can be accomplished consistently and promptly after metrizamide injection via lumbar or lateral cervical puncture. When lumbar puncture is used the patient's torso is oblique, but the neck is prone and straight and the table is tilted 20 degrees head-down to allow direct cervical filling with contrast material. E...
متن کاملTotal myelography with metrizamide through the lumbar route.
Myelography of the entire spinal cord was performed in 64 consecutive patients using metrizamide via lumbar puncture. Adequate to good opacification of the cervical and thoracic subarachnoid space was obtained in 63 patients. The technical factors responsible for this success rate are patient cooperation, high iodine concentration, high iodine concentration (250 mg/ml), slow injection rate, del...
متن کاملDelayed spinal subarachnoid hematoma: a rare complication of C1-C2 cervical myelography.
The lateral C1-C2 needle insertion for cervical myelography is a valuable alternative to lumbar puncture and is considered a safe procedure. Mullen et al. [1] initially described this approach for cervical cordotomy and later Kelly and Alexander [2] adopted this technique for visualization of the spinal canal and its contents. Mullen early reported transient paresis and vertebral artery embolus...

The side effects of iohexol were evaluated in the 300 patients who had nonemergency myelography over a 9 month period. No patients studied with myelography were excluded from the iohexol trial. Age range was 14-86 years. Introduction was by lumbar puncture in 206 patients and by lateral C1-C2 injection in 94. Side effects, including discomfort, were denied by 81.3% of the patients. The other 18...
